First Visit - 1 hour consultation/examination - $200.00 due at the time of initial visit. No insurance assignment will be accepted for the first visit fee. A receipt for your possible insurance re-imbursement will gladly be provided. Certain insurance companies, such as Medicare and Horizon Blue Cross and Blue Sheild do not cover this initial fee. Subsequent visits will range from $80.00 to $130.00, according to the type of treatment that is provided. If our office has a contractual agreement with your insurance company ie. Medicare, Qual Care, and Blue Cross/Blue Shield, then lower fees may apply. Insurance assignment will be accepted when eligibility has been determined on the patient's first visit. Any co-pay or co-insurance that is the patient's responsibility will be collected at the time of each visit. For our uninsured patients, convenient financing is available. |
